Lamotrigine is a voltage-gated sodium channel blocker:
Inhibits Glutamate Release: Suppresses high-frequency repetitive firing of neurons by blocking voltage-dependent sodium channels and stabilizing presynaptic neuronal membranes.
Seizure Control: Inhibits the excessive release of excitatory neurotransmitters (primarily glutamate), preventing seizure activity in the central nervous system.
Mood Stabilization: Stabilizes neuronal activity in cortical and limbic circuits, helping prevent future depressive relapses in bipolar disorder.
🚨 Critical Black Box Warning: Lamotrigine can cause severe, life-threatening skin rashes, including Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN).
The risk of severe rash is highest during the first 2 to 8 weeks of treatment or when starting at doses that are too high or increased too rapidly.
Seek emergency medical care immediately at the first sign of a rash, fever, swollen glands, or blistering in the mouth, eyes, or skin.
🚨 Rebound Seizures Warning: Do NOT stop taking lamotrigine suddenly.
Abruptly discontinuing antiepileptic medications can trigger severe rebound seizures or status epilepticus.
Discontinuation must always be done gradually over at least 2 weeks under direct medical supervision.
🚨 Drug Interaction Caution:
Valproate (Sodium Valproate): Significantly slows down lamotrigine metabolism, doubling its concentration and greatly increasing the risk of severe rash. Dose must be reduced by more than half if taken together.
Combined Estrogen Contraceptives: Lower blood levels of lamotrigine by up to 50%, which may reduce seizure or mood control.
Swallow Whole: Swallow tablets whole with a full glass of water.
Dosing Schedule: Take once or twice daily exactly as prescribed by your doctor.
Food: Can be taken with or without food.
Strict Dose Escalation: Always follow your physician's exact gradual titration (dose increase) schedule. Never skip steps or increase your dose faster than prescribed.
If you miss a dose, take it as soon as you remember unless it is almost time for your next dose. Do NOT double up on tablets to make up for a missed dose.
If you have stopped taking lamotrigine for more than a few days, contact your doctor before restarting, as you may need to re-titrate starting from a low initial dose to prevent rash.
Dizziness, headache, or unsteady movement (ataxia).
Double vision (diplopia) or blurred vision.
Drowsiness, fatigue, or insomnia.
Nausea, vomiting, or mild stomach upset.
Any new skin rash, hives, or skin itching.
Fever, flu-like symptoms, or swollen lymph nodes.
Unusual mood changes, increased depression, or suicidal thoughts.
Is Lamotrigine a narcotic or habit-forming drug?
No. Lamotrigine is an antiepileptic and mood stabilizer with no potential for physical addiction or abuse. However, it must never be stopped abruptly due to seizure risks.
Can I take Lamotrigine during pregnancy?
