Spiriva 2.5mcg Respimat is an advanced, prescription-strength, long-acting anticholinergic bronchodilator delivered via a propellant-free soft mist inhaler. Supplied as 60 puffs (1 cartridge with the Respimat reusable inhaler device), it is engineered to deliver a slow-moving, fine mist that penetrates deep into the lungs to keep airways open.
This prescribed maintenance treatment is recommended for the long-term, once-daily control and prevention of bronchospasm associated with:
Chronic Obstructive Pulmonary Disease (COPD): Including chronic bronchitis and emphysema.
Asthma: Used as a long-term maintenance add-on therapy in patients aged 6 years and older with persistent symptoms.
The active ingredient is Tiotropium Bromide:
Airway Relaxation: It blocks acetylcholine receptors in the smooth muscle of the airways, preventing bronchoconstriction.
24-Hour Bronchodilation: This keeps the airways relaxed and open for a full 24 hours, significantly reducing shortness of breath, wheezing, and daily respiratory flare-ups.
Emergency Use: Spiriva Respimat is a long-term maintenance medication. It should not be used to treat sudden, acute bronchospasm or sudden asthma attacks. Always keep a fast-acting rescue inhaler (such as albuterol) with you for sudden breathing emergencies.
Narrow-Angle Glaucoma & Prostate Issues: Be extremely careful not to spray the mist into your eyes, as it can precipitate or worsen narrow-angle glaucoma or cause acute eye pain and blurred vision. Inform your doctor if you have urinary retention or prostate enlargement before use.
Standard Dosing:
Inhale 2 puffs once daily at the exact same time each day.
How to Prime and Use the Respimat Inhaler:
Insert & Prime: Insert the cartridge into the inhaler device (first-time use only) and twist the base until it clicks. Flip the cap open, point down, and press the dose-release button until a mist is visible. Repeat until a spray is seen, then repeat 3 more times.
Turn: Keep the cap closed. Turn the clear base in the direction of the white arrows until it clicks (half a turn).
Open: Flip the cap completely open until it snaps.
Breathe Out: Breathe out slowly and completely, away from the inhaler.
Inhale: Close your lips around the mouthpiece without covering the air vents. Point the inhaler to the back of your throat. Press the dose-release button and breathe in slowly and deeply through your mouth.
Hold: Hold your breath for 10 seconds (or as long as comfortable).
Repeat the Turn, Open, Inhale, and Hold steps for the second puff. Close the cap.
While generally well-tolerated, potential side effects can include:
Dry mouth, sore throat, or mild throat irritation
Cough or mild upper respiratory tract infections
Rare urinary hesitation, rapid heart rate, or blurred vision if mist contacts the eyes
Store below 25°C or 30°C. Do not freeze the cartridge or inhaler.
Discard the cartridge 3 months after insertion, even if there is some solution left or it feels empty.
Keep strictly out of reach and sight of children.
Spiriva HandiHaler uses dry powder capsules loaded into a mechanical device, whereas Spiriva Respimat uses a liquid cartridge and a mechanical spring to generate a soft mist. Both deliver the same active medication (tiotropium) once daily.
The dose indicator on the inhaler shows approximately how many puffs remain. When the pointer enters the red scale area, there is enough medication for about 7 days (14 puffs) left.
